How to Fill Axis Bank Resident Consent Form to link Aadhaar

To link your Axis bank account with Aadhaar number you will need to fill up the Axis bank Resident Consent Form. The form can be obtained free of cost from the Axis branch or you can download it from above and take a printout on A4 paper.

To fill up the Axis Bank Resident Consent Form, take a blue pen and start filling up the form as per the below instructions.

By filling up the resident consent form to link Aadhaar with Axis Bank you will abide by the following conditions:

  • I hereby state that I have no objection in authenticating myself with Aadhaar based authentication system and consent to providing my Aadhaar number, Biometric and/or One Time Pin (OTP) data (and/or any similar authentication mechanism) for Aadhaar based authentication for the purposes Of availing Of the Banking Services from Axis Bank.
  • I understand that the Biometrics and/or OTP and/or any Other authentication mechanism I may provide for authentication shall be used only for authenticating my identity through the Aadhaar Authentication system for that specific transaction and for no other purposes.
  • I understand that Axis Bank shall ensure the security and confidentiality of my personal identity data provided for the purpose of Aadhaar based authentication.
  • I also hereby authorize to use my linked Aadhaar enabled bank account for receiving government payments across schemes that I am eligible using the Aadhaor based authentication.

If you agree with the above conditions then you can start filling up the form.

  1. The first field of the form is the Customer name, where you will need to fill your complete name
  2. In the second field of the form, you will need to fill your Axis bank account number
  3. The third field will be Customer ID, you need to enter your customer id. If you don’t know your customer id then you can know it by referring your passbook
  4. In the fourth field, enter your 12 digits Aadhaar number, make sure you enter your valid 12 digits Aadhaar number
  5. In the customer signature/ thumb impression field, you can do signature or you can leave a thumb impression by dipping your thumb into a stamp ink pad
  6. In the date field, fill up the date on which you are submitting the resident consent form

That’s it you are done with filling up the form, the branch certification field is for branch stamp and signature

You need to submit this resident consent form along with Aadhaar copy to the branch

The branch will stamp and signature in the branch certification space and take your request for the Aadhaar linking.
